We support individuals with disabilities to live independently in their communities.

Consultation and Support

We provide information about welfare systems and services as well as assistance for their daily living.

Networking in communities

We have been building networking systems to support daily living for people with disabilities in our community.  There are 3 Support Centers in this area, including Karashidane Center, which have been in charge of taking leaderships for this networking.

Our Main tasks include collecting and providing information about health and welfare for disables, promoting awareness, aggregating community issues and reflecting them in measures, and establishing collaborative structures for challenging cases.

Consultation and Support for those who are eligible for applying the services under the ‘Services and Supports for Persons with Disabilities Act’.

Care management services

We provide care management services by planning and coordinating the welfare services for people with disabilities.

Hospital discharge and community support

We support those who have been long-term residents of mental health facilities in smoothly discharging and continuing their living in the community.

*Note: Our center has full-time support specialists who have completed the following training:
Training of hospital discharge and community support which is for people with mentally disabled, conducted under the 2018 Ministry of Health, Labor and Welfare.
